By submitting this audition form you agree to the following Terms and Conditions: I am participating in online dance classes, during which I will receive information and dance instruction independent of the supervision I normally receive while taking class in the Studio. I recognize that dancing requires physical exertion, which may be strenuous and may cause physical injury, and I am fully aware of the risks involved. I understand that it is my responsibility to consult with a physician prior to my participation in online classes regarding any physical or mobility limitations or medical conditions that may prevent my full participation. I attest that the area I will use to participate in online classes will be free of furniture and other hazards that could impede my movement and safety. I understand that dancing on slippery floors and loose carpets is not safe.I agree to assume full responsibility for any risks, injuries or damages, known or unknown, I may incur as a result of participating in the online program. I agree that I will not hold CityDance, School and Conservatory staff or Teaching Faculty liable for any injuries sustained while taking online classes.I acknowledge that the School and Conservatory’s online dance classes are provided exclusively to students of CityDance School and Conservatory registered in the 2020-2021 season. I understand that recording of class content is strictly prohibited. I understand that music used in the videos is not property of CityDance and is for educational purposes only, and access to online content does not grant me a license or permission to use third party copyright materials. I acknowledge that CityDance will be monitoring acts of copyright infringement on the Teams platform. I agree to not reproduce, sell, share or distribute any recordings or screenshots of classes, and doing so will result in revocation of access to the online learning portal. In consideration of participation in online classes, I knowingly, voluntarily and expressly waive any claim I may have against CityDance and/or its agents for injury or damages that I may sustain as a result of participating in the program. Furthermore, I agree to release and hold harmless CityDance, Inc., its officers, directors, staff, employees, independent contractors, volunteers, and agents from any claims that may arise regarding the use of my image, including any claims of defamation, invasion of privacy, or infringement of moral rights, rights of publicity, or copyright. CityDance is permitted, although not obligated, to include my name as a credit in connection with my image/s. I understand that CityDance is a non-profit organization that promotes dance as an art form not only to its students, but also to the community at large. As such, articles, brochures, videos and websites may be used either promotionally or educationally and may include images of students or other participants in our programs. I hereby permit the use of my, or my child’s image to be photographed, videotaped or otherwise recorded for use in CityDance publicity or educational materials. These materials include, but are not limited to, photographs of classes and performances, newsletters, publications, and the CityDance website.I have read the above Release and Waiver of Liability and fully understand its contents. I voluntarily agree to the terms and conditions stated above.
